Disease Management

Middlesex Hospital has taken an aggressive approach to patient care that focuses on community–based, daily management of chronic disease by the patient, with the help of his or her primary care physician and a Middlesex Hospital/IRMA nurse case manager.

The goal of these programs is to help patients better manage their own disease in order to prevent acute episodes of their illness and to avoid emergency visits or inpatient admissions. These programs are designed so a physician can provide outpatient care, according to established standards for a patient’s specific disease.
